To use our form:

1. Save this PDF file to your computer desktop or another location where you will be able to find it.
2. Open the form and complete it.
3. SAVE THE FORM TO YOUR COMPUTER IN A LOCATION YOU WILL BE ABLE TO FIND IT.
4. If you have email software such as Outlook installed on your computer:
   Click on the submit button. Outlook or your other installed email software should open with an email addressed to us with the completed PDF attached. Add a message, if desired, and send.
5. If you DO NOT have email software such as Outlook installed on your computer:
   Create an email to orders@luersgroup.com. If the link doesn't work, copy and paste orders@luersgroup.com into your email.
6. Drag and drop (from the icon on your desktop or the file saved elsewhere) the saved order form into the email.
7. Please don't forget to include your contact information.
